During your first visit to Hammond & Proctor, we like to take you on a tour of our office before you're brought back to an exam room. We enjoy getting to show you all of the amazing dental technology that is unique to our office. We want you to get to know our friendly team of highly trained dental assistants and dental hygienists who will do everything they can to make you feel at home while providing a wonderful experience during your dental appointment.
New Patient Forms
Completing new patient forms is the quickest way for our team to understand your dental history as well as your overall well-being. With these forms we can have you registered as a new patient in no time. Our website provides a convenient way to complete new patient forms before your first appointment. But you can always come in and fill out the forms in our office if you prefer.

Receive Your Dental Cleaning
We provide a full oral exam (also known as a dental check-up) and take a full set of digital x-rays of your teeth to evaluate the current condition of your oral health and determine if there is any specific dental treatment that you can best benefit from.
If you are coming in to see us for a specific dental treatment because of an issue (such as oral pain), we will take you through all of the available options and help you decide what will be most effective option for correcting an issue and improving your overall oral health and cosmetic appearance.
